Every three years, providers are required to complete recredentialing through Valenz Health.

Here’s what to expect:

• Notifications start six months before your due date: Keep an eye out for mail or a fax from Valenz Health.

• Quick turnaround matters: Please return your form promptly with the following documents:

• DEA certificate

• Dental license

• Certificate of Insurance (COI)
